Zac Efron has reportedly been to rehab twice this year.

The 25-year-old actor appears to have checked himself into a clinic to help him battle what is believed to be a cocaine and alcohol addiction for a second time in April following a relapse, according to a new report.

Sources told gossip website TMZ.com that the hunk initially reached out for help to combat his apparent two-year struggle with class A drugs in March, but was forced to return in April when he began abusing the same substances while filming Seth Rogen's comedy 'Neighbors'.

Zac is said to have acted ''erratically'' causing concern among those who worked alonsdie him which led him to seek care again.

TMZ.com also put Zac's struggles down to his personal and professional problems, suggesting the star is disappointed that his film career has not had the same success as Disney's 'High School Musical' franchise which catapulted him into the spotlight.

In addition, it is believed that Zac is facing ''girl issues'' with sources saying was hanging out with ''a bad group of friends'', which he has since cut out of his life.
